Making an appointment with Summit Physical Therapy & Massage is as easy as ONE-TWO-THREE:

1.  Verify your Insurance Information

• A. Before you schedule an appointment with us, make sure we are contracted with your insurance company by calling the phone number that is listed on your insurance card.
  
• B.  Find out if you have outpatient physical therapy benefits and, if so, how much do they cover and ask whether prior authorization is required.

2.  Doctor’s Prescription
If your insurance company requires you to have a “referral”, “prescription”, or if the therapy has to be “medically necessary” then you will have to get a prescription from your doctor for physical therapy.

3.  Make an Appointment
